Our Services

Navigating the disability services system is overwhelming — OPWDD eligibility, the Front Door, choosing a Care Coordination Organization, Medicaid waivers, medical appointments, and pages of paperwork, often during an already stressful time. CCRI helps families understand the process, stay organized, keep appointments on track, and connect to the right resources.

This may include:

• Help understanding OPWDD eligibility and the Front Door process

• Guidance organizing and preparing application and supporting documentation

• Explanation of Care Coordination Organizations and how to choose one

• Coordinating and keeping track of medical, therapy, and service appointments — including help scheduling, organizing related paperwork, and making sure the logistics are handled

• Help identifying community resources, programs, and next steps

• Ongoing support so families aren't navigating alone

Please note: CCRI provides guidance, education, organization, and hands-on logistical support. We do not provide medical advice, make medical or treatment decisions, administer or manage medication, or provide nursing care or hands-on personal care, and we do not serve as your official OPWDD Care Manager or Care Coordination Organization. We help families stay organized and connected to the providers and agencies who deliver clinical care.

Connection and routine matter. CCRI provides accompanied community-based support that helps individuals participate in activities, build everyday skills, and stay engaged in their community rather than isolated at home.

This may include:

• Accompaniment to community activities, programs, and outings

• Support building routine, confidence, and social connection

• Encouragement of independence and meaningful community participation

Please note: This is community engagement and skill-building support. It does not include nursing care, medication administration, hands-on personal care, or unsupervised custodial care. Support is provided alongside the individual in community settings.

Transportation is one of the most common barriers families face. CCRI helps individuals and families arrange and access reliable transportation to appointments, programs, and community activities.

This may include:

• Help arranging and coordinating transportation options

• Accompaniment on transit to appointments and activities

• Reducing the logistical barriers that keep people from getting where they need to be

Please note: CCRI focuses on coordinating and accompanying transportation. Specific transportation arrangements are discussed and confirmed during your consultation based on each family's needs.

As CCRI grows, we are building toward quality day programming and person-centered, family-style residential opportunities (Individual Residential Alternatives) for adults with developmental disabilities in Suffolk County — directly addressing the critical shortage of personalized care options and long waitlists in our region. These services are part of our long-term vision and are not yet available, but they guide everything we build today.
